This gives a great performance boost;
- Sending 10 notifications before; ~6 seconds.
- Sending 10 notifications after; ~2 seconds.

This also requires reusing the same authentication token for each notification or else APNs will throw the TooManyProviderTokenUpdates error.

Introduces new APNSDeviceTokenInactive exception that can be acted upon to remove the device token from the database.

Oops...

`apns-push-type` will be `alert` if the body of the notification is set, otherwise it will be `background`.
